Are we clear about the mechanisms by which biopsy evidence of interstitial fibrosis following cardiac transplantation helps predict late post-transplant coronary artery disease?

چکیده

Transplant-associated coronary artery disease (TxCAD), or coronary vasculopathy, is the leading cause of death after the first year of transplantation and, once diagnosed, is associated with a mortality rate of 40% at two years (1). Though coronary vasculopathy is widely believed to represent some form of chronic humoral and cellular rejection, some nonimmune risk factors such as older donor age, ischemia time, dyslipidemia, preexisting donor coronary artery disease (CAD) and post-transplantation cytomegalovirus infection have also been identified (2,3). These risk factors suggest that both immune and nonimmune causes probably interact to lead to coronary vasculopathy, although the precise mechanisms involved remain poorly understood.

Cardiac angiotensin II receptors as predictors of transplant coronary artery disease following heart transplantation.

AIMS We tested the hypothesis that cardiac angiotensin II (Ang II) receptor gene transcription may predict the development of transplant coronary artery disease (TCAD) following heart transplantation. METHODS AND RESULTS We examined the gene transcripts of Ang II type 1 (AT1R) and type 2 receptors (AT2R) in endomyocardial biopsy specimens from 50 heart transplant recipients. The progression o...

Echocardiographic Evaluation of left Ventricular Function and Geometry in Pediatric Patients with Kidney Transplantation

Extended abstract Echocardiographic Evaluation of Left Ventricular Function and Geometry in Pediatric Patients with Kidney Transplantation Cardiovascular disease (CVD) is an important, leading cause of mortality and morbidity in patients with chronic kidney disease (CKD) as well as in renal transplant recipients. Cardiovascular complications become more important in children because of the i...

The management of post-cardiac transplantation coronary artery disease.

OBJECTIVE Allograft coronary artery disease remains the single greatest limitation to long term survival after cardiac transplantation. It is peculiarly aggressive in its behavior and diffuse in its nature. The role of conventional approaches to coronary artery revascularisation were studied in a selected group of cardiac transplant recipients. METHODS Of the 557 patients undergoing cardiac t...

Fibrogenesis: Mechanisms, Dynamics and Clinical Implications

Fibrosis is the pathological condition resulting in the growth of excess fibrous connective tissue in an organ or body system as a reparative or reactive process. In the field of clinical pathology, clinicians and medical scientists are endeavoring to translate experimental knowledge into effective, innovative treatments for a range of fibrotic conditions. The amelioration of whole organ functi...
